PRIMARY HEALTH CARE COUNCIL OF SOUTH EAST ONTARIO

Purpose

The purpose of the Primary Health Care Council (PHCC) is to provide collaborative leadership for the planning, delivery and evaluation of Primary Care services within the South East Local Health Integration Network (LHIN), creating a forum to address common issues pertaining to primary care across the continuum of health care.

Start-Up Organization

The "Founding Council" will serve for four years. It will be tasked with establishing linkages and standing committees with regional perspective and to provide a system for input to the LHIN. It will host an annual primary care forum. Toward the end of the initial term, a working group will be established to review organizational structure, terms of reference, and membership

Structure - Standing Committees

The PHCC will provide a forum for primary care groups to explore better access and integration of their service and to drive improved quality. Standing committees require approval of the Primary Care Council. Committees will be able to appoint a member to the Primary Care Council and request place on agenda as needed. The organizational structure and terms of reference of the PHCC reflect the active participation of the following groups:

Goals of Standing Committees
The goals of each standing committee are: provide a forum to address issues of common interest to primary care providers within this LHIN; to encourage involvement of primary care providers; to give "new life"  to some old structures in the region which may be struggling in regard to involvement; to promote access to primary care; to develop quality indicators; and to develop new "team" approaches to chronic care.

 

Working Groups

The Primary Health Care Council and its standing committees will facilitate the establishment of working groups as needed. These groups may be at an early stage of organization, tracking to become a standing committee, or they may have a defined short-term task. Working groups will have an opportunity to present to the PHCC or to have access to the Forum as appropriate. The PHCC will then provide feedback to the LHIN. The expertise of the PHCC will help foster linkages between the various community groups.
